Health and Welfare Benefits

DataClarity offers the Choice Plus plan from UnitedHealthcare.  Choice Plus is an "open access" health care plan. You can choose any provider you would like to see in the UnitedHealthcare national network. There are over 606,000 physicians and health care professionals and 5,015 hospitals nationwide. You must stay in the network to receive benefits. You have the freedom to choose your physician or specialist without visiting a "primary care physician" for a referral.  Choice Plus also offers you a level of benefits should you choose to seek care outside the network normally at a higher coinsurance, and/or deductible level. Choice Plus does not require you to designate a “primary care physician” to coordinate care, nor are specialist referrals required for eligible services.

Dental Benefits

Dental insurance is one of the most desired employee benefits. According to the 4th Quarter 2008 Principal Financial Well-Being Index, 70% of employees rate dental insurance as an important benefit.

DataClarity Corporation offers dental coverage from Principal Life.  In addition to having an extensive national network and a sophisticated claim system, Principal Life encourages preventive care to keep your teeth healthy, as well as help reduce future costly procedures and missed hours from work.

Principal Life provides dental care options to fit your specific needs

  • Our employer-paid dental insurance encourages cost sharing and provides flexible design options.
  • Principal Life also offers voluntary employee-paid coverage.

Network Options

Principal Life ensures proper credentialing and practice patterns of participating dentists.

  • Preferred Provider Organization (PPO) network where employees select the dentist of their choice and receive the advantage of increased benefits when choosing a participating dentist. Our PPO generates an average discount of 25%.
  • Exclusive Provider Organization (EPO) Dental is offered in select California markets

Term Life and Accidental Death and Dismemberment Insurance

Eligible employees receive the following life insurance benefits:

Basic Life Insurance

Basic life insurance in amount equal to $50,000.

Accidental Death and Dismemberment (AD&D)

AD&D insurance provides up to an additional $50,000 coverage for loss of life or injuries sustained within 90 days from the date of the accident.

UnitedHealthcare Pharmacy Management Program Plan

UnitedHealthcare’s pharmacy management program provides clinical pharmacy services that promote choice, accessibility and value. The program offers a broad network of pharmacies (more than 50,000 nationwide) to provide convenient access to medications.

While most pharmacies participate in our network, you should check first. Call your pharmacist or visit our online pharmacy service at www.myuhc.com. The online service offers you home delivery of prescriptions, ability to view personal benefit coverage, access health and well being information, and even location of network retail neighborhood pharmacies by zip code.

Supplemental Insurance

Eligible employees may also choose to elect supplemental insurance coverage for themselves and their spouse and unmarried dependent children. Employees may purchase these plans and benefits through pre-tax payroll deductions:

Flexible Spending Plan Accounts

Spending accounts allow employees to pay for certain expenses up to $5,000 annually before Federal income taxes and Social Security taxes are withheld, enabling employees to pay less in taxes and have more disposable income. Employees who are scheduled to work 30 or more hours per week are eligible to enroll on the first day of their first full calendar month of employment.

  • Dependent Care Flexible Spending Account - This benefit helps reduce employee's taxable income by allowing them to pay for eligible childcare expenses in pre-tax dollars per household (childcare receipts required).
  • Health Insurance Premium Conversion Account - Employees may pay for their share of DataClarity Corporation group health insurance premiums through pre-tax payroll deduction.

DataClarity 401(k) Retirement Plan

All employees may enroll in the DataClarity Corporation 401(k) Retirement Plan managed by Fidelity Investments. Employees may contribute up to 60% of their annual salary to the plan on a pre-tax basis to the IRS annual maximums.

The DataClarity 401(k) Retirement Plan has the following benefits and features:

  • No Probationary Period – Enroll on the first day of the following month after employment.

  • Rollovers Welcome - You can roll over assets from a previous employer's retirement plan or an IRA into the DataClarity Plan.

The DataClarity Corporation Stock Plan allows eligible employees the opportunity to benefit from long-term growth and the financial success of the company through the granting and purchase of DataClarity Corporation Incentive Stock Options. The granting of Incentive Stock Options to eligible employees through periodic stock option grants is based upon salary, responsibility, position and performance, and subject to Board of Directors' approval and program guidelines.

Vacation/Personal/Sick Days and Holidays

All regular, full-time employees are eligible for the following benefits:

Vacation/Personal/Sick Days

DataClarity employs a flexible system called Paid Time Off (PTO). PTO is an annual allowance of days that can be utilized for vacation, personal and sick days. During the first five (5) years of employment with DataClarity, eligible full-time employees accrue Paid Time Off (PTO) at the rate of fifteen (15) working days per year. At the beginning of the sixth year of employment, Paid Time Off (PTO) accrues at a rate of twenty (20) working days per year. Once PTO hours accrue on a monthly basis, there is no waiting period before they may be used. PTO hours reset at the beginning of each calendar year (January 1st), and therefore do not carryover from prior periods.

Holidays

In calendar year 2010, DataClarity Corporation will provide its regular, full-time employees with a total of nine (9) paid holidays including their birthday.
