Health Benefits

Welcome to Blue Options

With Blue Options, you can see in-network doctors for just a copayment. And you'll pay a deductible and coinsurance for other services.

You can be confident knowing that your plan offers you all of these services, plus:

With Blue Options, you can visit any provider in our network, and you'll be covered by your plan.  You can also visit out-of-network providers, but your coverage levels will be different. Log in to the Member Services section of bcbsnc.com to check your Member Guide for complete details.

  • Office visits - You can see any primary care doctor or specialist in our network for just a copayment.  And, annual precentive care is covered.  Additional covered services include: routine physical exams, immunizations, well-baby and well-child visits, OB/GYN exams, Pap smears and breast and prostate cancer screenings.
  • Hospitalization and outpatient care - When you're hospitalized or when you have outpatient procedures performed, you'll typically pay a deductible and coinsurance.  Coinsurance will apply after you've met your deductible.
  • Prescription drug coverage - You have access to our large network of pharmacies.  Use them and pay only a copayment or coinsurance.  Prescription drugs are covered at different levels, depending on the type of drug you purchase.  And, your presciption costs can be even lower when you select generic drugs.  For more details, check your Member Guide, or visit bcbsnc.com and select the "Find a drug" link.

Dental Benefits

Dental insurance is one of the most desired employee benefits. According to the 4th Quarter 2008 Principal Financial Well-Being Index, 70% of employees rate dental insurance as an important benefit.

DataClarity Corporation offers dental coverage from Principal Life.  In addition to having an extensive national network and a sophisticated claim system, Principal Life encourages preventive care to keep your teeth healthy, as well as help reduce future costly procedures and missed hours from work.

Principal Life provides dental care options to fit your specific needs

  • Our employer-paid dental insurance encourages cost sharing and provides flexible design options.
  • Principal Life also offers voluntary employee-paid coverage.

Network Options

Principal Life ensures proper credentialing and practice patterns of participating dentists.

  • Preferred Provider Organization (PPO) network where employees select the dentist of their choice and receive the advantage of increased benefits when choosing a participating dentist. Our PPO generates an average discount of 25%.
  • Exclusive Provider Organization (EPO) Dental is offered in select California markets

Vision Benefits

With Blue Options, you will receive an eye exam every year for a reasonable copayment, plus an annual allowance for eyeglasses or contact lenses.

In addition, you can take advantage of vision discounts available from Blue Cross and Blue Shield of North Carolina (BCBSNC) network optometrists as well as those in Davis Vision, Inc. nationwide network.

Discounts from BCBSNC providers

Receive a 30-percent discount on prescription eyewear including lenses, frames and non-disposable contact lenses.  Get a 15-percent discount on disposable contact lenses prescribed and purchased from any of our participating BCBSNC providers who own optical dispensaries.

Discounts from Davis Vision, Inc.

In addition to BCBSNC providers, we also offer vision discounts through Davis Vision, Inc. - one of the nation's leading managed vision care organizations.  You can get discounts on eye exams, frames, lenses and lens options, contact lenses and even non-prescription sunglasses.  Just present your BCBSNC ID card at a Davis Vision participating private practice provider or national retail location - such as Sears, Target, Wal-Mart and more - to start enjoying the savings.

Laser eye surgery discounts

Davis Vision offers corrective laser eye surgery discounts.  There are no claims to file - just show your BCBSNC ID card at the doctor's office.  Your program discount will include your consultation, the laser eye procedure, follow-up visits and any additional procedures for over/under correction.

Term Life and Accidental Death and Dismemberment Insurance

Eligible employees receive the following life insurance benefits:

Basic Life Insurance

Basic life insurance in amount equal to $50,000.

Accidental Death and Dismemberment (AD&D)

AD&D insurance provides up to an additional $50,000 coverage for loss of life or injuries sustained within 90 days from the date of the accident.

Supplemental Insurance

Eligible employees may also choose to elect supplemental insurance coverage for themselves and their spouse and unmarried dependent children. Employees may purchase these plans and benefits through pre-tax payroll deductions:

DataClarity 401(k) Retirement Plan

All employees may enroll in the DataClarity Corporation 401(k) Retirement Plan managed by Fidelity Investments. Employees may contribute up to 60% of their annual salary to the plan on a pre-tax basis to the IRS annual maximums.

The DataClarity 401(k) Retirement Plan has the following benefits and features:

  • No Probationary Period – Enroll on the first day of the following month after employment.

The granting of Incentive Stock Options to eligible employees through periodic stock option grants is based upon salary, responsibility, position and performance, and subject to Board of Directors' approval and program guidelines.

Vacation/Personal/Sick Days and Holidays

All regular, full-time employees are eligible for the following benefits:

Vacation/Personal/Sick Days

DataClarity employs a flexible system called Paid Time Off (PTO). PTO is an annual allowance of days that can be utilized for vacation, personal and sick days. During the first five (5) years of employment with DataClarity, eligible full-time employees accrue Paid Time Off (PTO) at the rate of fifteen (15) working days per year. At the beginning of the sixth year of employment, Paid Time Off (PTO) accrues at a rate of twenty (20) working days per year. Once PTO hours accrue on a monthly basis, there is no waiting period before they may be used. PTO hours reset at the beginning of each calendar year (January 1st), and therefore do not carryover from prior periods.

Holidays

In calendar year 2012, DataClarity Corporation will provide its regular, full-time employees with a total of eleven (11) paid holidays including their birthday.
